Use:
Put the number of your cite in each of the below entries like so, once per cite:

Put it next to your cited sentence. The number should increase by one as the page progresses.

title - Put the title of the page here (Such as 'Welcome to Neopets!')
Place this directly below your {{refheader|refconts=. Do one per cite. The number should match the cited sentence. Each should be directly under each other, starting from 1.
