Ever since neopets changed the rarity index and what each means, I've been lost. What exactly does it mean to have R100 now?

R100 are just rarer than R99s and restock less often.

Fun things to do on a Saturday Morning #1 - Recode Restocks! Yeah, we know that there have been problems with the restocks lately. It was basically due to it not being able to handle the number of different items that exist in Neopia. So the way it works has been completely rewritten and the following changes have been made...

Restocks now happen roughly one more time per hour
Slightly more of each type of item will go in each shop (more noticeably between rarity 65 and 85)
The absolute minimum price of an item has been changed from 20 NP to 50 NP
Items over rarity 90 now have a minimum price of 5,000 NP
Prices of all items have been upped a little (inflation!)
We found a bug where certain item types (like Petpets and weapons) had a really low chance of restocking, that has been fixed now!
Rarity 99 items are now 20% less likely to appear :)
UPDATE - Rarity 100 items can be stocked, but are 5 times less likely to appear than 99s! We will add more of these items soon as there are not currently that many.

so rarity 100 is rarer that rarity 99

Those are items that are special for some reason, and don't stock in normal shops, like omelettes. Rariety 180's are retired, and rariety 200's are hidden tower items.

Many of the items from Jhudora's Quests are rarity 130.

They never really changed the rarities. They just changed the programming for restocks. Various percentages and stuff like that.

rarity 100 are restock exclusive other than the gourmet food from the golden dubloon but i am not sure even if those restock in food shops. rarity 100 item are much much rarer..only just today/yesterday, a r100 coin restock..(Neopian Times Coin)
