Ever since I started playing 20 months ago, I do the four quests religiously--as often as they will let me--and have made quite a sum of nps doing so. Twice, I even reached the limit of 15 and got the "Sorry, no more quests for you today."
The key to the quests is to think of it in terms of economics--is the expected return greater than the cost I am putting out? You need to say to yourself, am I willing to risk this amount of nps to possibly get a toy sailboat as my prize (I have).
You need to set a limit for each of the questors and not do any quests over that limit. Here are my guidelines to how much I spend for each quest: 200 for the witch, 400 for the esophagor, 400 for the kitchen dude, 2000-2500 for the Snow Faerie. Some people say my limit for the kitchen dude is too low--however, I don't battle. If I did battle, I would probably spend more on the kitchen dude.
If you do the quest and bring back the item, return immediately to the questor to ask for another quest. If you decide not to do the quest, return to the questor as soon as your time limit is up.
I think the best questor right now is the esophagor. I have gotten some really nice stuff from him that I later resold for 15K-50K. Sometimes, he gives out just released books that, if you are quick, you can sell for a really good amount. I never do his quest together with the brain tree. I never, ever gotten anything worthwhile by combining the two--and wasted a lot of nps in the process.

when did she stop giving out r99 items? and why?
A long time ago. Late last year, I think. And probably because the TNT likes to nerf good things.
I believe the Snow Faerie stopped giving out really good items sometime last summer. Prior to the time, I had gotten many great things from the Snow Faerie--a sinsis sword, an iron lupe sword, a battle duck, a green scorchstone, a blue scorchstone, a really rare elephante battle item that I can't even remember, some morphing potions, and several items that hadn't yet been released in the shops, like the mosaic negg and McDonald's Fries, which I was able to turn around and sell instantly for 99,999 each.
However, the best I have gotten since then are things that I can resell for about 10,000-14,000K--a sweet and sour negg and a maple syrup negg are about the best things I can remember getting. The last time I did her quest, I got a healing potion X--woo hoo!!!, but at least the nps she gave me were more than I spent on the items she had asked for.
My guess as to why she stopped giving out really good things is because people were abusing the system--using multiple accounts to visit her so that they had a better chance of getting really good things from her.
Tested made this fabulous set for me!!! Isn't it great?
