3. What written guide do you guys recommend? IDNQ's guide is only for the first act, I really want a detailed guide I can refer back to if I get stuck, that goes right through the game.
4. How would you recommend playing the game different on evil compared to normal?
5. Same question as 4, but with insane.
Also, are there any other tips and tricks I should know about?

Evil: same as normal, mainly. This is slightly easier, because of the weapon drops from monsters (you get better weapons/armor at no extra cost). You might want to focus more on innates closer to the start of the game. Generally, as death is not fatal on evil, you can do whatever you want to and get away with it.
Insane is slightly more tricky: if you have the patience, it always pays off to overlevel. The hardest boss there is Zombom (since you only have Rohane to get him, and most people choose to fight him at Level 12 / 13). Concentrate on keeping mipsy alive, rather than getting a big gun at first. Surprisingly, Terask II was not that much harder compared to evil/normal. His extra HP makes little difference (as you can deal more damage than he heals), it will just be a slightly longer battle. Slow/Haste potions are recommended there (and probably everywhere, if you insist on not boring yourself to death fighting random monsters to gain levels).

Is there any particular penalty for other characters, for eg Mipsy dying? None of the things I've read seem very clear on this. Thanks for all the help
